We report a rare and diagnostically challenging case of a 39-year-old male patient who presented with symptoms of dizziness and headaches, without any focal neurological symptoms. Initial imaging studies suggested a germ cell tumor, and an endoscopic biopsy led to a preliminary diagnosis of a pineal parenchymal tumor of intermediate differentiation. However, histological evaluation following surgical resection revealed the final diagnosis to be an ectopic pituitary neuroendocrine tumor (PitNET), a condition that is exceedingly rare. Ectopic PitNETs are uncommon tumors that develop outside the normal anatomical location of the pituitary gland. Their atypical presentation often leads to misdiagnosis as other intracranial neoplasms. This case highlights the diagnostic challenges posed by ectopic PitNETs and contributes to the limited literature on this rare condition. It underscores the importance of maintaining a broad differential diagnosis in patients presenting with atypical intracranial neoplasms.

Glioblastoma (GBM), the most aggressive malignant tumour, is increasingly treated with immunotherapy. The stimulator of interferon genes (STING) pathway is key to tumour immunity and a studied target for immunotherapy. This study explores the immune landscape of GBM, focusing on spatial relationships between tumour-associated immune cells (TAICs) and STING-expressing cells, uncovering patterns linked to prognosis. We studied 14 recurrent GBM patients using protein composition and Gene Ontology (GO) analysis and analyzed immune pathways in 69 newly diagnosed GBM patients undergoing standard therapy. Spatial analysis of cells was performed using QuPath, CytoMAP, and R. The spatial distribution of immune cells and their proximity to STING-expressing (STING+) cells are associated with survival in glioblastoma (GBM). Specifically, a closer distance between STING+ cells and CD4+ cells was linked to improved survival, while greater distances between STING+ cells and CD11c+ cells also correlate with better outcomes. This suggests that spatial relationships, not just the presence of cells, are important for prognosis.

Tranexamic acid (TXA) is widely used to manage acute brain injuries, including subarachnoid hemorrhage, intracerebral hemorrhage, and traumatic brain injury. Despite its common usage, there is limited evidence on its safety in these conditions. We aimed to evaluate the impact of TXA on mortality and thromboembolic events in patients with acute brain injury. A systematic search of MEDLINE/PubMed, Embase, and the Cochrane Central Register of Controlled Trials was conducted from inception to May 2024. We included randomized controlled trials (RCTs) comparing TXA with placebo in patients aged 15 years or older with confirmed acute brain injury. Two reviewers independently assessed study quality using the revised Cochrane Risk of Bias tool and extracted data on patient demographics, intervention details, and outcomes, including mortality, thromboembolic events, and seizures. Meta-analyses were performed using random effects models. Twenty-five RCTs with 16,677 participants (8584 TXA, 8093 control) were included. The relative risk (RR) for overall mortality was 0.96 (95% confidence interval (CI) 0.91–1.03, p = 0.2433), indicating a nonsignificant difference between the groups, with no substantial heterogeneity (I2 = 0% [0–45%]). Additionally, no significant differences were observed in 30-, 90-, or 180-day mortality. The RR for total thromboembolic events was 1.11 (95% CI 0.97–1.28, p = 0.1236), indicating a nonsignificant difference between the groups, with low heterogeneity (I2 = 15% [0–51%]). Similarly, no significant differences were observed in the incidences of deep vein thrombosis or pulmonary embolism, ischemic stroke or transient ischemic attack, acute coronary syndrome or myocardial infarction, or seizures. However, the administration of TXA for more than 1 day was associated with a significant increase in thromboembolic events (RR 1.22, 95% CI 1.03–1.44). Administering TXA beyond 8 h of injury was also associated with a significant increase in thromboembolic events (RR 1.16, 95% CI 1.02–1.33). TXA administration does not significantly affect overall mortality or increase the risk of thromboembolic events in patients with acute brain injuries. However, prolonged use or delayed administration may be associated with an increased risk of thromboembolic events.

Although unilateral biportal endoscopy (UBE) has proven effective as a minimally invasive decompressive laminectomy for lumbar spinal stenosis, the learning curve for surgeons new to endoscopic techniques remains a significant barrier. This study aimed to quantify this learning curve and identify strategies to expedite proficiency, focusing on reducing operative times and complications. The cumulative summation test was used to evaluate the learning curves for both surgical methods.

Background: Serum calcium and magnesium levels are a key factor of the coagulation cascade and may potentially contribute to the pathophysiology of intracerebral hemorrhage (ICH) expansion. The aim of this study was to attain and sustain target levels of serum calcium and magnesium for three days following admission. Methods: A single-blind, prospective, multicenter randomized study was conducted from 2019 to 2022 years, enrolling acute ICH patients aged 18-80 years, with radiological diagnosis and without surgical intervention. The primary outcome was the expansion of ICH volume within the first 3 days between the study group and the control groups. Results: After implementing inclusion/exclusion criteria, 105 of 354 patients remained in the study. There were no significant differences in ICH volume on hospital days 2 and 3 between the groups. On the third day of hospitalization, admission serum magnesium levels showed a significant correlation with ICH expansion, whereas calcium levels did not. Conclusion: Admission serum magnesium levels were found to correlate with hematoma expansion in patients with acute ICH. While magnesium itself may not be a direct therapeutic target, it could serve as a valuable indicator for identifying potential therapeutic strategies aimed at preventing ICH volume increase.
