본 연구에서는 먹을 수 있는 중간사슬 지방산을 Brønsted 산 촉매로 활용하는 지속 가능한 수중촉매(aquacatalytic) 시스템을 보고하며, 놀라운 염석(salting-out) 효과를 이용해 “해수 중(on-seawater)” 조건에서 카보닐 알릴화(carbonyl allylation)가 가능함을 제시한다.

• An accelerated and intensive scalp acupuncture treatment is proven to be effective to improve the depressive symptoms and subjective quality of life among patients with depression. • Acupuncture is a neuromodulation intervention protocol that is well accepted by a multiethnic South-east Asia population. • The value of acupuncture as an augmentation therapy for in-hospital patients with primary depressive symptoms was examined via a randomized, pragmatic, controlled trial. To test the effectiveness of an accelerated acupuncture treatment as an augmentation therapy for patients with primary diagnosis of major depressive symptoms. Recruited patients were 1:1 randomly assigned to receive either treatment-as-usual (wait-list controlled group) or with additional acupuncture intervention (treatment group). The acupuncture treatment includes daily scalp acupuncture with manual stimulation for a total of 10 sessions conducted within 2-3 weeks. Blinded raters assessed patients’ depressive symptoms and quality of life at baseline (pre-treatment), immediately after treatment, 2 weeks, and 3 months post-treatment. A total of 84 patients with 59 (70.2%) Chinese, 17 (20.2%) Malay and 4 (4.8%) Indians participated in this trial. There is an overall improvement of depressive symptoms score assessed by Montgomery–Åsberg Depression Rating Scale (MADRS) and Quick Inventory of Depressive Symptomatology- Self report 16 items (QIDS-SR16), and an improvement of subjective Quality of life assessed by EQ5D Visual Analogue Score (VAS) and Utility Score (US) from pre-treatment to 3 months post-treatment for patients in both treatment groups and control group. In addition, compared to patients in control group, acupuncture induced an overall significantly better QIDS-SR16 (effect size η 2 = 0.15, p =0.025) and better EQ-5D Visual Analogue Scale score ( η 2 = 0.20, p =0.005). Acupuncture induced an accumulated and delayed antidepressant treatment effect for patients with depression. Our study informed the potential of an accelerated acupuncture treatment as an augmentation antidepressant therapy within a multiethnic South-east Asian population.

서론: 양극성 장애는 기분 변화(우울 및 (저)조증 삽화)를 특징으로 하는 만성 정신과 질환이다. 우울은 질병 부담에 상당한 기여를 하며, 환자의 3분의 1은 치료에 반응하지 않는다. 본 시험은 치료저항성 양극성 우울증(TRBD)을 앓는 환자에서 가속 간헐적 세타 버스트 자극(Accelerated intermittent Theta Burst Stimulation, aiTBS)의 효과를 연구하고자 하였다. 연구 설계: 본 연구는 양성자 개념(open-label) 증거-개념(proof-of-concept) 시험으로, 양극성 장애 I형 또는 II형을 가진 TRBD 환자 8명이 포함되었다. aiTBS는 좌측 배외측 전전두피질(left dorsolateral prefrontal cortex, dlPFC)에 적용하였으며, 초당 5버스트[5 Hz]로 전달하되, 각 버스트는 50 Hz의 3개 펄스를 포함하였다. 참가자들은 세션당 1800 펄스를 받았고, 연속 5일 동안 하루 1회씩 총 8회의 세션을 진행하였다. 자극은 휴지 운동역치(resting motor threshold, rMT)의 100%에서 10분간 시행하였고, 세션 간 간격(intersession interval, ISI)은 50분이었다. 주요 결과: 주요 결과는 예비 효능, 안전성 및 내약성이었다. 효능은 해밀턴 우울 평가 척도(Hamilton Depression Rating Scale, HDRS-17)에서의 평균 감소로 표현하였고, 안전성은 (중대한) 이상반응으로, 내약성은 부작용 및 중도탈락률로 평가하였다. 결과: 제출 시점에 7명의 환자가 연구를 완료하였다. 3일차 HDRS-17 점수의 평균 감소는 7.0(p0.006**), 5일차 11.14(p0.014*), 2주 후 12.29(p0.009**), 4주 후 9.43(p0.07), 3개월 후 9.57(p0.14)이었다. 흔한 부작용은 피로(86%)와 불편감(86%)이었다. 두 명의 환자는 이러한 부작용으로 인해 전체 프로토콜을 완료하지 못하였다. 한 명은 자극 강도를 낮추어(90% rMT) 시행하였고, 다른 한 명은 40개 세션 중 7회를 건너뛰었다. 결론: 본 예비 연구는 좌측 dlPFC에 대한 aiTBS가 TRBD에서 잠재적으로 안전하고 내약성이 있으며 효과적인 치료가 될 수 있음을 시사한다. 그러나 제한된 표본 수와 3개월 후 높은 재발률은 추가 연구가 필요하며, 유지(유지요법) 전략을 함께 탐색해야 한다.
