윤소정 연구실은 의학교육 분야에서 교육심리 및 임상 현장 학습의 측정·평가·교육설계를 수행합니다. 임상실습에서 학습자의 성찰 수준을 확인하기 위해 설문도구의 타당도와 신뢰도를 분석하고, 초기 임상노출(ECE)의 현황과 교육적 요구를 조사하여 교수·평가 요소를 정리합니다. 또한 COVID-19 및 입학 전환기와 같은 상황에서 의대생의 스트레스, 적응, 실패두려움 변화를 잠재 프로파일로 분류해 지원 필요 지점을 도출합니다. 아울러 의사 역량 관련 문헌을 텍스트 네트워크 분석과 토픽 모델링으로 구조화하여 연구 동향을 파악하는 방법론 연구도 병행합니다.

Current status and educational needs of early clinical exposure in Korean Medical Schools: A cross-sectional survey study

Objective: To examine the status of early clinical exposure (ECE) programs in Korean medical schools and to determine the educational needs for ECE in undergraduate medical education. Methods: In this cross-sectional study, 30 medical education experts and 65 professors from 30 medical schools across Korea were surveyed about the status of ECE programs in medical schools and the educational needs for ECE. This survey was conducted between January and March 2024 using Google Forms, and the collected data were analyzed using frequency analysis. Results: Out of the 30 participating medical schools, 70% had implemented ECE programs. In most schools, ECE programs were required courses and offered from the first year of pre-medical phase to the second year of medical phase. Among 95 respondents, 88.4% recognized the necessity of ECE programs. Most respondents considered "understanding the role of a doctor" an essential objective (70 out of 95, 73.7%) and outcome (73 out of 95, 76.8%) of ECE programs. Most respondents considered "observation/field trips" and "reflection journal" as essential teaching methods (69 out of 95, 72.6%) and assessment methods (68 out of 95, 71.6%) in ECE programs, respectively. Furthermore, most respondents (68 out of 95, 71.6%) considered the "lack of educational support personnel" a concern in implementing ECE programs in medical schools. Conclusions: This study sheds light on the status of ECE programs in Korean medical schools. Additionally, its results regarding the educational needs for ECE have implications for the future implementation of ECE programs in Korean medical schools.

Core shared competencies between physicians and nurses in primary care: a Delphi study in Korea

PURPOSE: This study aimed to identify core shared competencies required for effective physician-nurse collaboration in primary care. METHODS: A three-round Delphi survey was conducted from November 2024 to February 2025 with 30 experts (12 physicians, 18 nurses), including family medicine professors, primary care physicians, nursing professors, and practicing nurses. Experts evaluated the importance and roles of interprofessional team approaches using online questionnaires. Quantitative analyses included mean, standard deviation, and content validity ratio (CVR). RESULTS: The first round confirmed the necessity of interprofessional teamwork in cases such as chronic disease management, rehabilitation, elderly care, and mental health. Essential team members were physicians, nurses, and social workers, with additional professionals engaged as needed. Through iterative consensus, six shared competencies were derived: (1) patient-centered integrated care, (2) treatment plan development and implementation, (3) communication and collaboration, (4) professional development as a team member, (5) Evaluation and feedback on service outcomes, and (6) disease prevention and health promotion. All items met consensus criteria (CVR ≥0.34). CONCLUSION: These findings clarify physician-nurse shared competencies in primary care and provide a foundation for developing competency-based interprofessional curricula and training programs to enhance collaborative care quality and patient outcomes.

A Delphi Study to Validate the Patient-Centered Doctor’S Competency Framework in Korea

Defining a competent doctor is important for educating and training doctors. However, competency frameworks have rarely been validated during the process of their development in Korea. The purpose of this study was to validate the patient-centered doctor’s competency framework, which had been developed by our expert working group (EWG). Two rounds of Delphi questionnaire surveys were conducted among a panel of experts on medicine and medical education. The panel members were provided with six core competencies, 17 sub-competencies, and 53 enabling competencies, and were asked to rate the importance of these competencies on a 5-point Likert scale. Between April and July 2021, a total of 28 experts completed both rounds. The data of the Delphi study were analyzed for the mean, standard deviation, median, inter-rater agreement (IRA), and content validity ratio (CVR). A CVR >0.36 and IRA ≥0.75 were deemed to indicate validity and agreement. This study found that five enabling competencies were not valid, and agreement was not reached for three sub-competencies and two enabling competencies. In consideration of CVR and the individual opinions of panel members at each session, the final competencies were extracted through consensus meetings of the EWG. The competencies were modified into six core competencies, 16 sub-competencies, and 47 enabling competencies. This study is meaningful in that it proposes patient-centered doctor’s competencies enabling the development of residents’ milestone competencies, an assessment system, and educational programs.
